Apply for Service

To begin the process of applying for services, first complete the LIFE Incorporated Service Application and Release of Records Exchange Forms. These documents require participant and/or legal guardian signature and must be completed prior to service delivery.

We also strongly encourage a Pre-Service Interview with our Management Team to discuss the specifics of your case.

Once we’ve received the completed Service Application and Releases, we may request additional documentation to conduct a thorough assessment to determine if our agency is an appropriate placement. This step is essential to ensuring adequate resources are available and quality care is provided. Please feel free to ask questions and/or provide input during the assessment period.

The entire intake process takes approximately 30-60 days; however, shorter transitions can be accommodated in emergency situations and will be handled on a case-by-case basis.
