Residential Habilitation: Supported Living Program
 
Supported Living is a service available through the Home- and Community-Based Services (HCBS) Waiver, which includes residential habilitation services and supports. The Supported Living – Residential Habilitation model provides needed assistance to adults living in his or her own residence (i.e. apartment, residential house, duplex, etc.) on a twenty-four (24) hour basis. LIFE, Inc. provides hourly residential habilitation and/or daily rate services and supports.

  • What is Residential Habilitation under the Supported Living Model?

    Residential Habilitation provides assistance to adults living with developmental disAbilities in the areas of self-care, receptive and expressive language, learning, mobility, self-direction, capacity for independent living, and economic self-sufficiency in the community and residential settings.

    The purpose of this service is to provide the individual with the most normalized, inclusive, community-based living arrangement; most heightened level of independence possible; and, highest quality of life possible. Individuals are able to reside in their own home with twenty-four (24) hour supports to ensure health and safety needs are addressed and monitored; financial obligations are monitored; and, rehabilitation and habilitation activities are addressed on a daily basis.

  • Why is this service important?

    In 1994, The Home- and Community-Based Services (HCBS) Waiver for adults living with Developmental DisAbilities was added to the Idaho Medicaid Program. This program allows individuals who otherwise qualify for “institutional level of care” to access equivalent services and supports in the community.

  • What services are available in Idaho under the HCBS Waiver?

    Residing in Group Homes; Residential Habilitation (i.e. Certified Family Home or Supported Living); Independent Living; Adult Day Care; Community Supported Employment; Targeted Service Coordination; Respite Care; Behavior Consultation; Non-Medical Transportation; Chore service; Home Deliver Service; Nursing Services; therapy services; emergency response services; and transportation system.

  • How do I pay for rent, utilities, food, clothing, services, etc.?

    The twenty-four (24) hour services and supports provided by LIFE, Inc. are reimbursed through Medicaid funding. If you qualify for Medicaid, your services will be approved through the Department of Health and Welfare and must be prior authorized. Rent, food, clothing, spending money, etc. are paid by you, the individual, and/or through monies received from Social Security monthly payments (SSI) to the individual - you.

    LIFE, Inc. offers Representative Payee services free of charge to Supported Living participants! It is your decision to choose a Representative Payee to assist in managing monies; LIFE, Inc. is one option.
