Residential Habilitation: Agency Affiliation

 

Agency Affiliation is a service available through the Home- and Community-Based Services (HCBS) Waiver, which includes residential habilitation services and supports. Certified Family Home – Residential Habilitation model provides needed assistance to adults living in a provider’s residence on a twenty-four (24) hour basis. Certified Family Homes are required to affiliate with a Residential Habilitation Agency to maintain certification.  

What is Agency Affiliation for Certified Family Home?

Residential Habilitation provides assistance to adults living with developmental disAbilities in the areas of self-care, receptive and expressive language, learning, mobility, self-direction, capacity for independent living, and economic self-sufficiency in the community and residential settings.

The purpose of this service is to provide the individual with a community-based living arrangement, in which he/she resides in the provider’s home and is assisted with regular access to the community (e.g., medical appointments, employment, etc.). Individuals are able to reside in a Certified Family Home with twenty-four (24) hour supports to ensure health and safety needs are addressed and monitored; financial obligations are monitored; and, rehabilitation and habilitation activities are addressed on a daily basis.

How do I find a Certified Family Home?

You are responsible for locating a Certified Family Home, along with members of your Person-Centered Planning Team. LIFE, Inc. does, however, post openings in our Administrative Offices, as they become available. Once a Certified Family Home has been located, LIFE, Inc. will assist the provider during the initial certification process, as well as all future re-certifications.

What services are available in Idaho under the HCBS Waiver?

Residing in Group Homes; Residential Habilitation (i.e. Certified Family Home or Supported Living); Independent Living; Adult Day Care; Community Supported Employment; Targeted Service Coordination; Respite Care; Behavior Consultation; Non-Medical Transportation; Chore Service; Home Deliver Service; Nursing Services; therapy services; emergency response services; and transportation system.

Do Certified Family Homes charge rent, utilities, food, clothing, services, etc.?

LIFE, Inc. Agency Affiliation services are reimbursed through Medicaid! The twenty-four (24) hour services and supports provided by Certified Family Homes are also reimbursed through Medicaid funding. If you qualify for Medicaid, your services will be approved through the Department of Health and Welfare and must be prior authorized.

Room and board are paid by you, the individual, and/or through monies received from Social Security monthly payments (SSI) to the individual - you. Additional monies for clothing, spending money, etc. are also your responsibility.
